Participation Release: In signing up to participate in the Spooktacular Workout Weekend, I release and discharge Life Addicts LLC, Xtreme Hip Hop and its agents or employees from all such claims, demands, injuries, damages, actions or causes of action, from all acts of active or passive negligence on the part of Life Addicts LLC, Xtreme Hip Hop and its agents or employees. I authorize Life Addicts LLC, its subsidiaries and representatives to use my picture (electronic, print, photographic, motion, and video images) as well as my voice including video and sound recordings for promotional purposes.
